Information on health checkups for infants and toddlers
On the day when the baby was just 100 days old, a text message was sent to the number 1577-1000. The message was a text message sent by the National Health Insurance Service, informing that an infant checkup slip had been sent. Fill out the 1st Infant Health Checkup Questionnaire
To fill out the questionnaire, first access the National Health Insurance website. (link below)

Log in using your public credentials. If you are not a member, you must register as a member first.
After logging in, you will see a screen like the one below.
If you are filling out the medical questionnaire for the first time, your baby may not be registered as a member of My Family.
To register your baby as a family member, click [My Page] in the upper right corner.
When you go to the My Page screen, find the [Family member application] button and click it.
After entering the baby’s social security number, the baby’s name will be displayed in the space called Family Members, as shown in the picture above.
After registering the baby, go to the first place and click on “Health Checkup Target (Interview)/Result Search” in the [Individual] tab.
When you go to the healthy person’s page, click [Create Web Medical Questionnaire] in the menu on the left, click the [Infant information] tab, and then click the [Create Medical Questionnaire] button to fill out the questionnaire.
It is said that you can print out the written munjinpyo and take it to the checkup institution, or apply for a reading service so that you can read it on the web.
You can apply for the medical questionnaire viewing service in the [Personal Information] tab in the last picture above.
It is said that you must remember the password (4 digits) you entered when applying for the service and inform it to the examination agency.
Tomorrow, Saturday, my baby and my wife are going to visit a designated hospital nearby to get a health checkup for the baby. I wonder what the top (or bottom) percentage of my baby’s height, weight, and head circumference are at 145 days old.
